Understanding Peptic Ulcers: What Causes Them and How They Develop

A peptic ulcer is a sore that forms in the lining of your stomach, small intestine, or esophagus. Think of it like a small wound that develops when the protective layer of mucus breaks down. The stomach naturally produces strong acid to help digest food, but when that protective coating wears away, the acid can damage the tissue underneath, creating these painful sores.

For many decades, doctors believed that stress and spicy foods caused most ulcers. We now know this isn't accurate. Research over the past 30 years has shown that the vast majority of ulcers—about 90 percent—result from one of two main causes. The first is a bacterium called Helicobacter pylori, or H. pylori. The second is regular use of nonsteroidal anti-inflammatory drugs, commonly called NSAIDs, which include aspirin and ibuprofen.

H. pylori is particularly common. According to the American College of Gastroenterology, this bacteria infects the stomach lining of roughly one in three people worldwide. In the United States, the infection rate is lower, affecting about 10 to 15 percent of the population. The bacterium spreads through contaminated food or water, or from person to person through saliva or feces. Many people carry H. pylori without ever developing an ulcer, but in some cases, it triggers inflammation that leads to ulcer formation.

NSAIDs work by reducing inflammation and pain, which makes them popular for treating headaches, arthritis, and muscle aches. However, long-term use can wear away the stomach's protective mucus layer. People over 65 who take NSAIDs regularly face a significantly higher risk of developing ulcers. Even younger people who use these medications frequently may experience problems.

Less common causes include severe stress (stress ulcers), smoking, and rare conditions like Zollinger-Ellison syndrome, which causes the stomach to produce excessive acid. Understanding what caused your ulcer matters because the treatment approach differs depending on the cause.

Practical takeaway: If you develop persistent stomach pain or think you might have an ulcer, write down your symptoms, any medications you take regularly (especially NSAIDs), and any recent illnesses. This information will help your doctor determine the underlying cause.

Recognizing Ulcer Symptoms and When to Seek Medical Attention

The most common symptom of a peptic ulcer is a burning stomach pain, typically felt between your navel and breastbone. This discomfort can range from mild to severe and may feel like heartburn. For some people, the pain comes and goes over weeks or months. For others, it's more constant. The pain often occurs when your stomach is empty—between meals or at night—and may improve temporarily after eating or taking antacids.

Beyond burning pain, ulcers can cause other digestive symptoms. Some people experience bloating, a feeling of fullness even after eating small amounts, heartburn (acid reflux), nausea, or a loss of appetite. In some cases, people notice their stools appear dark or tarry, or they vomit material that looks like coffee grounds. These last two symptoms are particularly important because they suggest bleeding in the stomach or upper intestine, which requires medical attention.

It's crucial to understand that not everyone with an ulcer experiences obvious symptoms. Some people have "silent ulcers" and don't notice anything wrong until a serious complication develops. This is why older adults and people taking NSAIDs regularly should discuss their risk with a doctor even if they feel fine.

You should seek medical care if you experience severe abdominal pain that comes on suddenly, persistent vomiting, vomit containing blood, black or tarry stools, unexplained weight loss, or appetite changes lasting more than a week. These symptoms may indicate complications such as perforation (a hole in the ulcer) or severe bleeding. In these cases, go to an emergency room rather than waiting for an appointment with your regular doctor.

Even if your symptoms seem mild, it's worth mentioning them to your doctor during your next visit. Untreated ulcers can develop serious complications. About 10 to 15 percent of people with peptic ulcers develop bleeding that requires treatment. Another 5 to 10 percent experience perforation, where the ulcer creates a hole in the stomach or intestine lining.

Practical takeaway: Keep a symptom diary for one to two weeks before your doctor appointment. Note when pain occurs, what you ate before it started, how long it lasts, and what relieves it. This information helps your doctor narrow down the cause.

Diagnostic Tests: How Doctors Confirm an Ulcer and Identify the Cause

Your doctor can't diagnose an ulcer just by listening to your symptoms and examining your abdomen. Several testing methods may be used to confirm whether you have an ulcer and what caused it. Understanding these tests helps you know what to expect and why your doctor might order them.

The most direct way to see an ulcer is through an endoscopy. In this procedure, a doctor inserts a thin, flexible tube with a small camera through your mouth and down into your stomach and small intestine. This allows the doctor to see the ulcer directly and, if needed, take a small tissue sample for testing. While endoscopy sounds uncomfortable, doctors typically use sedation to keep you relaxed and pain-free during the procedure. It takes about 15 to 30 minutes, though you'll need someone to drive you home afterward because of the sedation.

If your doctor suspects H. pylori, several tests can detect this bacterium. A urea breath test involves swallowing a special substance and then breathing into a collection bag. The lab analyzes your breath to detect if H. pylori is present. A stool antigen test checks a sample of your stool for evidence of the bacteria. Blood tests can detect antibodies your immune system creates in response to H. pylori infection. During an endoscopy, the doctor can take a biopsy (small tissue sample) and test it directly for the bacterium.

Upper gastrointestinal X-rays, sometimes called upper GI series or barium swallow, were once the standard way to detect ulcers. While less common now that endoscopy is available, some doctors still use this method. You drink a chalky barium solution, and X-ray images track how it moves through your digestive system, revealing ulcers and other abnormalities.

If you're taking NSAIDs regularly, your doctor may test you for H. pylori even if you don't have symptoms. Research shows that treating H. pylori in NSAID users significantly reduces ulcer risk. Your doctor might also test your stomach acid levels in certain situations, particularly if they suspect Zollinger-Ellison syndrome or other rare conditions.

Practical takeaway: Before any diagnostic procedure, ask your doctor to explain what will happen, why they're ordering the test, and when you'll get results. If you're anxious about endoscopy, let your medical team know—they can discuss sedation options with you.

Treatment Options: Medications and Approaches for Ulcer Healing

Modern ulcer treatment is highly effective, with most ulcers healing within four to six weeks of starting appropriate medication. The specific treatment approach depends on what caused your ulcer. If H. pylori is responsible, the goal is to eliminate the infection. If NSAIDs caused the ulcer, the approach involves stopping the medication (when possible) and allowing the ulcer to heal while protecting the stomach lining.

For H. pylori infection, doctors typically prescribe a combination of medications called triple therapy or quadruple therapy. Triple therapy usually involves two antibiotics plus a medication that reduces stomach acid, taken for one to two weeks. Common antibiotic combinations include amoxicillin with clarithromycin, or amoxicillin with metronidazole. Quadruple therapy adds a fourth medication called bismuth subsalicylate. This combination approach works because H. pylori can develop resistance to single antibiotics, but resistance to multiple medications simultaneously is rare. Studies show that properly administered combination therapy successfully eliminates H. pylori in about 90 percent of cases.

Regardless of the ulcer's cause, acid-reducing medications play a crucial role in healing. These fall into two main categories.
