Understanding Tendonitis: What Happens to Your Tendons

Tendonitis is inflammation of a tendon, which is the thick cord of tissue that connects muscle to bone. When a tendon becomes irritated or inflamed, it causes pain, swelling, and sometimes limits your movement. This condition can happen in almost any tendon in your body, but it most often affects the shoulders, elbows, wrists, knees, and heels.

Tendons are made of strong fibrous tissue designed to handle repeated stress and movement. However, when you overuse a tendon or injure it, tiny tears can develop in the tissue. Your body responds to these small injuries by sending inflammatory cells to the area. This inflammation is actually part of the healing process, but it also causes the pain and swelling you feel. Unlike acute injuries that happen suddenly from a fall or direct impact, tendonitis typically develops gradually over weeks or months.

The most common types of tendonitis include tennis elbow (lateral epicondylitis), which affects the outer part of the elbow; golfer's elbow (medial epicondylitis), which affects the inner elbow; and Achilles tendonitis, which affects the tendon running down the back of your heel. Rotator cuff tendonitis affects the shoulder and is especially common in people who throw or do overhead activities. Studies show that about 2% of the general population develops some form of tendonitis each year, with rates higher among athletes and people in physically demanding jobs.

Risk factors for developing tendonitis include age (tendons become less flexible with age), repetitive motions, poor technique during activities, sudden increases in activity level, and certain medical conditions like rheumatoid arthritis. The pain typically feels like a dull ache that gets worse with movement and improves with rest. Some people describe a creaking or grating sensation when moving the affected area.

Practical Takeaway: Recognize that tendonitis develops when repeated stress or injury causes inflammation in the tissue connecting muscle to bone. Understanding your symptoms and how they started can help you describe your condition to a healthcare provider and identify what activities may be causing the problem.

When to Seek Medical Evaluation for Tendon Pain

Not all tendon pain requires a doctor's visit, but certain situations warrant professional evaluation. If you experience severe pain that comes on suddenly, or if pain persists for more than two weeks despite rest and over-the-counter treatment, scheduling an appointment with your primary care doctor or a specialist makes sense. You should also seek evaluation if the pain limits your ability to perform daily activities, if swelling is severe or spreading, or if you have numbness or tingling along with the pain.

Healthcare providers diagnose tendonitis through a combination of methods. Your doctor will ask about your symptoms, how long you've had them, what activities make them worse, and whether you've had similar problems before. They will perform a physical examination, moving and testing the affected area to identify where the pain originates and how much movement is possible. Ultrasound and MRI imaging can show inflammation and small tears in the tendon, though not all cases require imaging. X-rays are sometimes taken to rule out bone fractures or other structural problems.

Different specialists handle tendonitis depending on which tendon is affected. For shoulder and rotator cuff issues, an orthopedic surgeon or sports medicine doctor may be appropriate. For heel pain, a podiatrist or orthopedic specialist can help. Your primary care doctor can perform an initial evaluation and provide referrals to specialists if needed. Some people start with a sports medicine doctor, especially if the tendonitis is related to athletic activity or repetitive work.

During your evaluation, your provider will want to know about your work activities, hobbies, exercise routine, and any recent changes in activity level. Be specific about when pain occurs—during or after activity, at rest, at night—and what movements make it worse or better. This information helps your provider determine the cause and develop an appropriate treatment plan. Bring a list of any medications or supplements you take, as some can affect healing or interact with treatments.

Practical Takeaway: Schedule a medical evaluation if tendon pain is severe, persists beyond two weeks, or interferes with daily function. Prepare for your appointment by noting when your pain started, what makes it worse, and any activities that seem connected to your symptoms.

Conservative Treatment Options for Tendonitis

Most cases of tendonitis respond well to conservative treatment, meaning treatment without surgery. The first-line approach is often called RICE, which stands for Rest, Ice, Compression, and Elevation. Rest means limiting or stopping the activity that caused the tendonitis. This doesn't necessarily mean complete immobility—in fact, gentle movement is often better than complete rest—but it means avoiding the specific motions or activities that trigger pain. Ice should be applied for 15 to 20 minutes at a time, several times daily, especially in the first few days after symptoms start or after activities. Wrap ice in a thin cloth rather than applying it directly to skin. Compression using an elastic bandage or sleeve reduces swelling, and elevation of the affected area (holding your arm above heart level, for example) also helps with swelling.

Over-the-counter nonsteroidal anti-inflammatory drugs (NSAIDs) like ibuprofen and naproxen can reduce both pain and inflammation. Taking these medications as directed on the package can help you tolerate gentle movement and therapy better. Acetaminophen reduces pain but doesn't address inflammation. Some people find topical creams containing NSAIDs helpful, as these deliver medication directly to the affected area with less systemic effect. However, it's important to follow dosing instructions and not use NSAIDs long-term without discussing it with your doctor, as extended use carries risks.

Physical therapy is a cornerstone of tendonitis treatment and recovery. A physical therapist teaches you specific exercises designed to gently strengthen the muscles around the injured tendon and improve flexibility. Early-stage exercises focus on gentle range-of-motion work and pain-free movement. As healing progresses, exercises become more challenging, building strength gradually. Eccentric exercises—where you lengthen a muscle under tension—have strong evidence for treating certain types of tendonitis, particularly Achilles and elbow tendonitis. A physical therapist will modify exercises based on your progress and pain levels.

Bracing, strapping, or taping can reduce stress on the injured tendon during healing. An elbow strap worn just below the point of pain can relieve pressure on the affected tendon. Heel cups or special shoe inserts help with Achilles tendonitis. These devices work by changing how force travels through the tendon or by reducing the range of motion in ways that decrease pain. Your doctor or physical therapist can recommend the right type of support for your specific situation.

Practical Takeaway: Conservative treatment combining rest, ice, anti-inflammatory medications, and guided physical therapy resolves most cases of tendonitis. Consistency with exercises and activity modification matters more than seeking quick fixes.

Advanced Treatment Options When Conservative Care Isn't Enough

When conservative treatment doesn't produce improvement after six to twelve weeks, or in cases of more severe tendonitis, doctors may recommend additional interventions. Corticosteroid injections deliver strong anti-inflammatory medication directly to the injured tendon area. These injections can provide significant pain relief and reduce inflammation, often allowing better participation in physical therapy. However, steroid injections work best for short-term relief rather than long-term healing, and there are limits to how many can be given in a particular area, as repeated injections can potentially weaken tendon tissue over time.

Platelet-rich plasma (PRP) therapy and stem cell therapy represent regenerative medicine approaches that are becoming more common. PRP involves taking a sample of your own blood, processing it to concentrate platelets and growth factors, then injecting this concentrate into the damaged tendon. The theory is that these growth factors stimulate healing. Stem cell therapy uses cells that can develop into various tissue types to promote repair. While these treatments show promise in research, evidence for their effectiveness is still developing, and they are often not covered by insurance. These procedures are typically performed by orthopedic specialists or sports medicine doctors.

Extracorporeal shock wave therapy (ESWT) uses high-energy shock waves directed at the injured tendon. This treatment is thought to stimulate the body's healing response. It has the most evidence supporting its use for calcific tendonitis and some cases of Achilles tendonitis.
