Understanding What Causes Earaches and Why They Hurt

An earache is pain in or around the ear that can range from mild discomfort to severe throbbing. According to the American Academy of Otolaryngology, earaches affect millions of people each year, with ear infections being one of the most common reasons adults visit urgent care clinics. Understanding what causes your earache is the first step toward finding relief at home.

Common causes of earaches include ear infections, buildup of earwax, sudden changes in air pressure, temporomandibular joint (TMJ) problems, sinus infections, and minor injuries to the ear canal. Swimmer's ear, medically known as otitis externa, develops when water becomes trapped in the ear canal and bacteria or fungi grow. Middle ear infections, or otitis media, occur when fluid builds up behind the eardrum. Outer ear infections affect the skin of the ear canal itself.

Temperature changes play a role too. Cold weather can make earaches worse because blood vessels constrict, reducing circulation to the ear. Flying or diving creates pressure changes that can cause discomfort lasting several hours or even days. Clenching your jaw from stress or dental problems can trigger referred pain in the ear, meaning the actual problem originates elsewhere but you feel pain in your ear.

Earaches in children versus adults present differently. Children may tug at their ears, cry inconsolably, or develop fever, while adults might experience muffled hearing or a feeling of fullness. The Eustachian tube, which connects the middle ear to the throat, is shorter and more horizontal in children, making them more prone to fluid accumulation.

Practical Takeaway: Identify whether your earache is sharp and sudden, dull and persistent, or accompanied by other symptoms like fever or discharge. This information matters when deciding whether home care is appropriate or medical attention is needed. Keep a brief note of when the pain started, what makes it worse, and any recent events like swimming or air travel.

Warm and Cold Therapy Methods for Pain Relief

Temperature-based treatments are among the most effective home remedies for earache relief. The logic is straightforward: warmth increases blood flow and relaxes muscles, while cold numbs pain signals. Research published in the Journal of Emergency Medicine shows that temperature therapy can reduce pain intensity by up to 30 percent in some cases. Most people notice relief within 5 to 15 minutes of applying either warm or cold therapy.

For warm therapy, a heating pad set on low or medium is one option, but you should never apply it directly to the ear without a barrier. Instead, wrap the heating pad in a thin towel and hold it against the ear for 10 to 20 minutes. A warm compress works similarly: soak a clean cloth in warm (not hot) water, wring out excess moisture, and hold it against the affected ear. A microwaveable heat pack designed for therapeutic use also provides consistent warmth. Some people fill a clean sock with uncooked rice, tie it closed, microwave it for 1 to 2 minutes, and use it as a reusable heat pack.

Cold therapy numbs the area and reduces inflammation. Apply an ice pack wrapped in a cloth—never directly on skin—to the outside of the ear for 10 to 15 minutes at a time. You can create a simple ice pack by filling a plastic bag with ice and wrapping it in a dish towel. A bag of frozen vegetables wrapped in cloth also works. Some people alternate between warm and cold therapy every 15 minutes, which can provide better relief than either method alone.

The choice between warm and cold often comes down to personal preference and what caused the earache. Warm therapy typically feels more comforting for muscle tension or chronic earaches, while cold therapy works better for acute inflammation or injury. Experiment with both to see what provides the most relief for your situation. Never apply heat or cold for longer than 20 minutes without a break, as this risks skin irritation.

Practical Takeaway: Keep a heating pad and a reusable ice pack in your home. Start with whichever method feels most soothing, apply for 10 to 15 minutes, take a 5-minute break, and reapply as needed throughout the day. Track which method works best for you so you know your preference for future earaches.

Over-the-Counter Medications and Topical Treatments

Many over-the-counter medications can reduce earache pain and inflammation when used correctly. Ibuprofen and acetaminophen are oral pain relievers that work differently: ibuprofen reduces inflammation, while acetaminophen blocks pain signals. According to the CDC, these medications are generally safe when used at recommended dosages, though you should follow label instructions carefully and never exceed the daily maximum dose.

Topical anesthetic drops designed for ear pain contain ingredients like benzocaine, which numbs the ear canal. These drops work quickly, often providing relief within minutes. However, they should only be used if the eardrum is not perforated. If you have discharge, bleeding, or suspect your eardrum might be damaged, avoid using drops and see a healthcare provider. Read labels carefully, as some ear drops are specifically formulated for infection prevention rather than pain relief alone.

Decongestant sprays or oral decongestants may help if your earache is related to sinus congestion or Eustachian tube dysfunction. These medications reduce swelling in the nasal passages and can allow fluid to drain from the middle ear. However, decongestants should not be used for more than three consecutive days, as prolonged use can cause rebound congestion, making symptoms worse. Always check ingredient lists if you take other medications, as some decongestants can interact with heart medications or blood pressure medications.

Natural topical treatments like mullein flower oil, garlic oil, and tea tree oil have been used traditionally for ear pain. While some people report relief, scientific evidence supporting their effectiveness is limited. If you choose to use these, ensure they are specifically prepared for ear use and avoid putting anything directly into the ear canal unless you are certain the eardrum is intact. Applying these oils to the outer ear or a cotton ball placed at the ear opening may provide some comfort without risk of complications.

Practical Takeaway: Keep a bottle of age-appropriate pain reliever (ibuprofen or acetaminophen) and ear drops on hand. Before using ear drops, confirm that your eardrum is not perforated. If you've had ear surgery or suspect a perforated eardrum, skip drops and stick with oral pain relievers and temperature therapy instead.

Positioning and Lifestyle Adjustments for Comfort

How you position your body can significantly impact earache severity. When pain is in one ear, sleeping on the opposite side keeps pressure off the affected ear and allows gravity to help drain fluid. Prop yourself up with pillows at a 45-degree angle rather than lying flat, as this elevation also promotes drainage and reduces pressure in the Eustachian tube. Many people find that sitting upright provides more relief than lying down, which is why earaches often feel worse at night when you're horizontal.

Avoid inserting anything into the ear canal, including cotton swabs, fingers, or small objects. The ear canal is delicate, and inserting objects risks pushing wax deeper, scratching the canal, or puncturing the eardrum. If you must clean your ears, use a soft cloth on the outer ear only. Let wax work its way out naturally—it serves a protective purpose by trapping bacteria and dust before they reach the eardrum.

Environmental adjustments matter too. Protect your ears from sudden temperature changes by wearing a hat or earmuffs in cold weather. If you swim or shower, prevent water from entering the ear canal by tilting your head to allow water to drain out afterward. Some people place a cotton ball lightly coated with petroleum jelly in the ear opening while showering as a water barrier. If water does get trapped, tilt your head and gently tug on your earlobe downward to help it drain.

Manage stress and tension, which can worsen jaw clenching and referred ear pain. Gentle stretches for the neck and jaw, relaxation techniques, and adequate sleep support overall pain management. Avoid loud noises and wear ear protection if exposed to loud environments, as sound sensitivity often increases when you already have ear pain. Limit time with earbuds or headphones, and keep volume at moderate levels even when your ears feel fine.
