Understanding Ingrown Toenails: What Causes Them and Why They Happen
An ingrown toenail occurs when the edge or corner of a toenail grows into the skin beside it, rather than growing straight out. This condition most commonly affects the big toe, though it can happen to any toe. The nail becomes embedded in the skin, creating an opening where bacteria can enter and cause infection. According to podiatric research, ingrown toenails affect approximately 10% of the population at some point in their lives, with certain groups experiencing higher rates.

Several factors contribute to ingrown toenail development. The most common cause is improper nail trimming—cutting nails too short or at an angle rather than straight across encourages the nail edge to grow into surrounding skin. Tight shoes and hosiery that put pressure on the toe can also force the nail inward. Some people have a natural genetic predisposition to ingrown toenails due to the shape and curvature of their nails. Toenail injuries from stubbing, dropping something on the foot, or sports-related impacts can damage the nail matrix, causing it to grow abnormally. Certain medical conditions, including fungal infections and diabetes, increase the risk of developing ingrown toenails.
Age plays a role as well. Teenagers and young adults tend to experience ingrown toenails frequently, possibly because their nails are growing rapidly. Older adults may also develop them due to changes in nail texture and increased brittleness. People with poor circulation or numbness in their feet, such as those with diabetes, may not notice an ingrown toenail developing until it becomes severely infected.
Practical Takeaway: Understanding the root cause of your ingrown toenail helps you prevent future occurrences. If your case resulted from tight shoes, switching to properly fitting footwear is essential. If improper trimming caused it, learning correct nail care techniques can prevent recurrence.
Recognizing Symptoms Early: When to Seek Professional Care
Early recognition of ingrown toenail symptoms allows for intervention before the condition worsens. Initial signs include redness and mild discomfort around the nail edge. You may notice the skin becoming tender when you apply pressure, such as wearing tight shoes or walking. Some people experience a slight swelling that makes the toe feel tight in footwear.

As an ingrown toenail progresses, symptoms intensify. Pain becomes more pronounced, especially when walking or wearing closed-toe shoes. The area around the nail may develop increased redness and warmth to the touch. Pus or clear fluid may drain from the site, indicating infection has developed. You might notice a small amount of bleeding when you touch or bump the area. The skin around the nail can become inflamed and tender to even light pressure.
Certain warning signs indicate you should see a healthcare provider rather than attempting home treatment. If you have diabetes, circulatory problems, or a weakened immune system, professional evaluation is important because your body may have difficulty fighting infection in this area. Signs of infection—including pus, significant warmth, spreading redness, or a red streak extending up the toe—require professional attention. Severe pain that prevents normal walking or activity also warrants medical evaluation. If the ingrown toenail doesn't improve after one to two weeks of home care, or if symptoms worsen, scheduling an appointment with a podiatrist or healthcare provider is recommended.
Practical Takeaway: Keep a simple symptom log for the first week of home treatment. Note changes in pain level, redness, swelling, and any discharge. This information helps you track whether the condition is improving or if professional care is needed.
Immediate At-Home Care Steps: Relieving Pain and Preventing Infection
When an ingrown toenail first develops, several at-home measures can reduce pain and support healing. Soaking the foot in warm water is one of the most effective initial treatments. Mix warm water with Epsom salt—typically one-half to one cup of salt per gallon of water—and soak the affected foot for 15 to 20 minutes. Perform this soak two to three times daily, particularly in the morning and evening. The warm water reduces inflammation and softens the skin around the nail, making the area less tender. Epsom salt has properties that may reduce swelling and support the body's natural healing response.

After soaking, carefully dry the foot completely, paying special attention to the area between the toes where moisture can linger. Moisture retention increases infection risk, so thorough drying is important. While the skin is still soft from soaking, you can gently work to lift the nail edge away from the skin using a clean, thin implement like a sanitized toothpick or dental floss. Insert it carefully under the nail edge—this should not cause sharp pain. Slide a small piece of clean cotton or dental floss under the nail to create a gentle barrier between the nail and skin. This positioning encourages the nail to grow over the skin rather than into it. Replace this barrier daily after soaking.
Pain management during the healing process supports your comfort. Over-the-counter pain relievers like acetaminophen or ibuprofen can reduce discomfort and inflammation. Follow package directions for dosing and frequency. Ibuprofen, in particular, has anti-inflammatory properties that may help reduce swelling around the affected area. Wearing loose, open-toed shoes or sandals reduces pressure on the toe and decreases pain when walking. Avoid any activities that put stress on the affected foot, such as running or contact sports, until healing is well underway.
Practical Takeaway: Create a daily care routine: morning soak with Epsom salt, dry thoroughly, insert a cotton barrier under the nail edge, and apply pain relief as needed. Evening care should include another soak and barrier replacement. Consistency with this routine over one to two weeks often shows improvement.
Nail Care and Prevention Techniques to Avoid Future Problems
Proper nail trimming is the single most important factor in preventing ingrown toenails from returning. Toenails should be trimmed straight across, not curved or at an angle. Use clean, sharp nail clippers designed specifically for toenails—regular fingernail clippers are too small and can slip, causing irregular cuts. Cut the nail so it extends slightly beyond the surrounding skin rather than cutting it flush with or below the skin surface. The nail edge should be visible above the skin on all sides. Most people need to trim their toenails every two to four weeks, though this varies based on how quickly nails grow.

After trimming, use a nail file to smooth any sharp edges. Rough or jagged edges can snag on socks and cause minor trauma that may lead to ingrown toenails. File gently in one direction rather than sawing back and forth. If you struggle with proper self-trimming due to vision problems, limited mobility, or other concerns, having a podiatrist or healthcare provider trim your nails removes the risk of improper technique.
Footwear choices significantly impact ingrown toenail development. Shoes should fit properly with adequate room in the toe box—you should be able to wiggle your toes comfortably. When standing, there should be about one thumb's width of space between your longest toe and the end of the shoe. High heels and narrow-toed shoes concentrate pressure on the toes and increase ingrown toenail risk. Choose shoes with a wider toe box and lower heel when possible. Socks should also fit properly without being too tight around the toes or foot. Cotton socks that absorb moisture are preferable to tight synthetic materials.
Protecting your toes from injury helps prevent ingrown toenails caused by trauma. Wear appropriate footwear during activities like sports, yard work, or hiking. Be cautious when walking barefoot to avoid stubbing toes or dropping objects on your feet. If you do experience a significant toe injury, monitor the area closely for signs of nail damage or abnormal growth.
Practical Takeaway: Schedule a nail trimming session this week using proper technique—straight across, leaving the edge visible above skin, with smoothed corners. Make this your standard practice every month. Evaluate your shoe collection and set aside at least one pair with a roomy toe box for daily wear, especially if you're currently dealing with an ingrown toenail.
