Understanding Head Lice: What They Are and How Infestations Happen
Head lice are small insects that live on the scalp and feed on human blood. These parasites are about the size of a sesame seed, measuring roughly 2-3 millimeters in length. They come in tan, gray, or brownish colors and move quickly across the scalp. Head lice cannot fly or jump; they spread only through direct contact with an infected person's hair or by sharing personal items like combs, hats, pillows, or headphones.

According to the Centers for Disease Control and Prevention (CDC), head lice infestations affect millions of people in the United States each year. While head lice can affect anyone regardless of age or hygiene habits, children ages 3 to 11 years old experience the highest rates of infestation. Contrary to common misconceptions, lice do not indicate poor cleanliness. In fact, lice prefer clean hair to dirty hair, as it allows them to move more easily to the scalp where they feed.
An infestation begins when a single pregnant female louse reaches a new host. She can lay 4 to 8 eggs per day, and these eggs, called nits, hatch within 7 to 10 days. The nits are oval-shaped and about the size of a pinhead. They attach firmly to hair shafts, usually close to the scalp where it is warm. Within 9 to 12 days after hatching, young lice reach maturity and can reproduce, creating a cycle that can quickly spread an infestation throughout a household or classroom.
Recognizing the signs of head lice early makes treatment more effective. Common symptoms include itching on the scalp, neck, or behind the ears; a tickling sensation in the hair; small red bumps or sores on the scalp from scratching; and the presence of nits on hair shafts. Some people have no symptoms at all during the early stages of infestation. This is why parents and caregivers should regularly inspect the scalps of children, especially after they have spent time in group settings like schools, camps, or sports activities.
Practical Takeaway: Head lice are spread through direct contact with infected hair or shared personal items. Check your child's scalp weekly for nits and signs of infestation, particularly if they attend school or participate in group activities. Early detection allows for quicker treatment and reduces the risk of spreading lice to other family members or classmates.
Over-the-Counter Treatments: What Options Are Available
Over-the-counter (OTC) lice treatments are widely available at pharmacies and drugstores without a prescription. These products contain active ingredients designed to kill lice and, in some cases, nits. The most common active ingredients include permethrin, pyrethrins, and dimethicone. Each works differently, and understanding these options helps you make an informed decision about treatment.

Permethrin is a synthetic insecticide commonly found in products labeled for treating head lice. It works by disrupting the nervous system of lice, causing paralysis and death. Permethrin-based treatments typically come in 1% concentration for over-the-counter use. These products are generally applied to dry hair, left on for a specific time period (often 10 minutes), and then rinsed out. The CDC notes that permethrin has been used for decades and remains an effective treatment option, though some populations of lice have developed resistance in certain regions.
Pyrethrins are natural insecticides derived from chrysanthemum flowers. They work similarly to permethrin by attacking the nervous system of lice. Many over-the-counter products combine pyrethrins with piperonyl butoxide (PBO), which enhances the insecticide's effectiveness. These products are typically applied to dry hair, left for 10 minutes, and then rinsed. Because they work on a similar mechanism to permethrin, lice that have developed resistance to permethrin may also resist pyrethrin-based treatments.
Dimethicone is a silicone-based oil that works differently from chemical insecticides. Instead of targeting the nervous system, dimethicone coats the louse and blocks its respiratory system, causing death by suffocation. Products containing dimethicone may be applied to wet or dry hair and often require longer contact times, sometimes 15 to 20 minutes or even overnight application. Because dimethicone uses a physical mechanism rather than a chemical one, lice resistance is less likely to develop.
Other OTC options include products containing benzyl alcohol, which also suffocates lice, and natural oil-based treatments such as tea tree oil or coconut oil. While these natural options are less studied than permethrin or pyrethrins, some people prefer them due to concerns about chemical exposure. However, their effectiveness varies, and they may require longer application times or repeated treatments.
Practical Takeaway: Read product labels carefully to identify the active ingredient and follow application instructions exactly. If one treatment does not work after two applications spaced 7 to 10 days apart, consult a healthcare provider about alternative options. Different active ingredients work through different mechanisms, so switching to a product with a different ingredient may be more effective if resistance is present.
Prescription Treatments for Resistant or Severe Infestations
When over-the-counter treatments fail to eliminate head lice, prescription medications offer stronger alternatives. These prescription-strength treatments are recommended by healthcare providers when lice show resistance to standard OTC products or when infestations are particularly severe. Common prescription options include ivermectin, benzyl alcohol, and spinosad, each with different active ingredients and application methods.

Ivermectin is an oral medication that has gained prominence in treating head lice in recent years. It works by paralyzing and killing lice and their nits. The typical prescription involves two doses taken by mouth, spaced 7 to 10 days apart. According to studies published in medical journals, ivermectin shows effectiveness rates of 95% or higher when used as directed. This oral approach eliminates the need for topical application and can be particularly useful for young children or individuals with thick or long hair that is difficult to treat with topical products. However, ivermectin is not suitable for all patients; pregnant women and children under a certain weight threshold cannot use this medication.
Spinosad is a prescription topical treatment derived from soil bacteria. It is applied to dry hair, left for 10 minutes, and then rinsed out. One application of spinosad often eliminates lice, though a second application may be recommended after 7 days if nits remain. Spinosad is particularly useful against permethrin-resistant lice strains. Studies show that spinosad is effective even when applied for only 10 minutes, making it simpler to use than some other treatments that require longer contact times.
Benzyl alcohol is available in a prescription-strength formulation (Ulesfia lotion) at higher concentrations than some OTC products. It works by suffocating lice and requires two treatments spaced 7 days apart. This medication is safe for infants and young children, making it an option when other treatments are not suitable. However, it does require precise timing and repeated applications, which some families find challenging.
Malathion is an older prescription insecticide that remains effective, particularly against lice resistant to other treatments. It has a strong odor and requires careful application, including avoiding open flames and heat sources. Despite these limitations, malathion remains useful in cases where other treatments have failed. Healthcare providers may recommend malathion as a last-resort option when multiple other treatments have not been effective.
Practical Takeaway: Schedule an appointment with a healthcare provider if over-the-counter treatments have not worked after two applications. Bring information about which OTC products you have already used so your provider can recommend a prescription alternative with a different active ingredient or mechanism of action. Prescription treatments may cost more than OTC options, so ask about coverage through your insurance plan.
Non-Medicated Treatment Methods and Home Remedies
Many families explore non-medicated approaches to treating head lice, either as primary treatment or as supplements to medicated products. These methods range from combing and suffocation-based treatments to oils and heat-based approaches. While non-medicated methods generally require more time and patience than medicated treatments, they appeal to families concerned about chemical exposure or seeking additional treatment options.
